| Form C-28 has been revised to encompass all theclassifications that has been approved for use by the Departmentof Labor. This attached form is to be used for all submissionsafter April 1, 1996. The code number that coincides with eachtitle on this form and in our computer files are not in numericalorder; the titles have been alphabetized within the given craftand operator categories to aid the contractor and DEOC in workingwith the forms. Each title has been allotted five (5) wagereporting slots with the exception of Construction Worker I(Skilled Laborer) and Construction Worker II (Laborer). Both ofthese titles have been given ten (10) wage reporting slots. Ifmore than five (5) wage reporting slots are necessary for asingle classification, additional wage rates should be reportedon the reverse side of each form. Similar procedures should beused as required on the previous Form C-28. The wage rate datawill be processed by Information Systems Division using the samefile format as on the previous form. | |
